背景

ACE阻害剤に不感応し,アンジオテンシンII(Ang II)からアンジオテンシン-(1-7) [Ang-(1-7) ]を高効率で形成するACEのホモログであるアンジオテンシン変換酵素(ACE)2を,リシノプリル,ロサルタン,両剤併用による慢性遮断に対する応答性を検討した。

方法

成体のルイスラット36匹を割り付け、2週間、これらの薬剤を飲料水に添加して投与し、動脈圧、水分摂取量、尿量を試験期間中記録した。腎排泄変数の測定には、アンジオテンシンI(Ang I)、Ang IIおよびAng-(1-7)の排泄率の評価が含まれ、試験終了時に採血した血液は血漿アンジオテンシン濃度の測定に使用された。腎皮質からのサンプルを、半定量的逆転写酵素(RT)リアルタイムポリメラーゼ連鎖反応(PCR)により、レニン、アンジオテンシノーゲン(Aogen)、ネプリリシン、アンジオテンシン1型および2型(AT(1)およびAT(2))およびmas受容体mRNAについてアッセイした。ACE2活性は、Ang IIのAng-(1-7)への変換速度として決定した。

結果

リシノプリルまたはロサルタンを投与したラットでは、同等の血圧低下が得られたが、両薬ともより大きな動脈圧の低下を示した。また,リシノプリルとロサルタンを投与したラットでは,いずれの治療法でも多飲多尿が認められたが,クレアチニン排泄量には変化は認められなかった。リシノプリルは血漿中のAng IとAng-(1-7)のレベルと尿中排泄率を増加させたが、血漿中のAng IIは減少したが、尿中のAng IIには影響を与えなかった。ロサルタンは血漿中および尿中のAng-(1-7)に同様の変化を示したが、尿中のAng-II排泄に変化はなく、血漿中のAng-IIを増加させた。併用療法では、リシノプリルで得られた血漿中および尿中のAng IおよびAng-(1-7)レベルの変化と同様の効果が得られた。腎皮質のAogen mRNAは、リシノプリルまたは併用療法のいずれかを投与されたラットで増加したが、3つの治療法はすべて、レニンmRNAの強力な増加をもたらした。一方、ACE、ACE2、ネプリリジン、AT(1)およびmas受容体のmRNAは、3つの治療法すべてで変化しなかった。腎皮質ACE2活性は、リシノプリルまたはロサルタンを投与されたラットでは有意に増加したが、併用投与されたラットでは変化しなかった。

結論

我々のデータは、リシノプリルまたはロサルタンを投与されたラットの腎臓の腎膜で測定された ACE2 活性の増加に主に反映されるように、正常血圧の低いラットの腎臓における Ang II からの Ang-(1-7) の形成における ACE2 の役割を明らかにした。データはさらに、ACE阻害またはAT(1)受容体遮断後の動物の尿中のAng-(1-7)レベルの増加は、ヘプタペプチドの腎内形成を反映していることを示している。

感想

ロサルタン(リシノプリルも)について、ACE2受容体に選択性がありそうという論文を読んでみました。動物実験であることもあり実臨床での効果とは切り離して考える必要がありますが、なかなか興味深い内容でした。
